Important results from the project
The purpose of My Care Pathways was to increase patient empowerment and to enhance the pre-requisites to achieve a common view about the individual´s health between patients, care providers and relatives. The goal was to create innovative eHealth services, enabling patients to administer and manage their own journey through healthcare, and to provide methods and guidelines allowing for development of similar eHealth services in co-production between different parties. These goals have been fulfilled through timely delivery according to plan and within the given time frame.
Expected long term effects
The project outcome consists of three main deliveries: 1) the e-service My Care Pathway which enables patients to track their journey through healthcare, 2) the security service API-Gateway and an open data platform to create innovation from new e-services, and 3) a Software Development Kit with tools for app developers to produce innovative eHealth services. The results enhance the prerequisites to achieve a holistic view of the individual care process. A pilot test of the component ´Track your referral´ showed positive response from patients and care providers.
Approach and implementation
The project was performed as a national project with broad support and close collaboration between care givers, patient representatives, county councils, universities, patient organisations and industry. A total of 13 project partners contributed. The work was managed within four tracks: care, technology, research and My Care Contacts with high cross-track collaboration to assure optimal results, e.g. through quarterly workshops and collaboration with national fora. Internationally the project was acknowledged by the European Commission.
