Important results from the project
Our aim and targets of the projects have been met according to plan. This has been measured through conversations, surveys and meetings as well as collecting statistics from the system. Improvement, however very little, of symptoms and quality of life has been reported but may be due to the "novelty factor". Fewer visit to health care has been made since some visits have been taken place "virtual". Extension of this project has been decided and will provide a fuller picture.
Expected long term effects
The result and expected benefits of the project have been overall good and according to expectations. Monitoring of vital parameters and creating a disease comprehension through information and self-measurements have worked well and been appreciated by both staff and patients. The use of the forum has been less than thought. This is probably due to the fact that the patient group have been small and that changing the behavior around asking others for help takes time to mature. A few visits to primary care have been avoided thanks to the system.
Approach and implementation
-Installation and training takes more time than expected. -Behavior changes takes time, even more so among this age category. -The computer knowledge vary significantly, so the efforts will be larger in some cases than foreseen. -This can definitely be widely implemented and achieved with good results. -Positive response from both patients and staff
